چکیده

In free-flap surgery, if the appropriate length of the pedicle and the distance necessary for separation of the artery and vein are determined earlier during flap elevation, the operative time and the stress placed on the flap pedicle can be reduced. However, the pedicle of a flap passes through 3-dimensional structures, particularly in head and neck reconstructions, and because of the difference in body shapes between individuals, even experienced microsurgeons may misjudge the required distance. To date, no easy method to counteract this problem has been reported. Accordingly, we have developed a technique for estimating the required pedicle length necessary for safe vascular anastomosis, as well as the distance necessary for separation of the artery and vein, by using a flap model with vascular tapes. Between June 2014 and April 2016, we applied our technique to 12 patients undergoing head and neck reconstruction and 1 patient undergoing breast reconstruction. In the 13 cases in which the present technique was used, we could estimate the length of each pedicle smoothly and efficiently. Furthermore, considerable excess or deficiency did not occur in the vascular anastomosis. The present technique is recommended as it allows the effective tailoring of the flap pedicle in free-flap surgery.

ANATOMY OF THE SUPERFICIAL INFERIOR EPIGASTRIC ARTERY FLAP

ABSTRACT Background: Several case studies have described the use of the superficial inferior epigastric artery (SIEA) flap as a pedicled flap for reconstruction of upper and lower ex tremities, or a free fasciocutaneous flap when a large amount of skin coverage is required for hemifacial atrophy, breast or head and neck reconstruction. Apparently, the anatomical findings of previous studies ar...

Acute choke zone effects: Lessons from radioactive and fluorescent microspheres in a pig model muscle flap.

BACKGROUND Choke vessels dilate and contract to regulate blood flow between adjacent arterial angiosomes. In skin flap surgery, when arterial inflow to an angiosome is ligated, choke vessels allow blood supply from an adjacent angiosome. In muscle flap surgery, the vascular anatomy is analogous to skin flaps; however, while it is established that the choke vessels will fully dilate irreversibly...

Scalp Free Flap Reconstruction Using Anterolateral Thigh Flap Pedicle for Interposition Artery and Vein Grafts

We experienced satisfactory outcomes by synchronously transplanting an artery and vein using an anterolateral thigh flap pedicle between the vascular pedicle and recipient vessel of a flap for scalp reconstruction. A 45-year-old man developed a subdural hemorrhage due to a fall injury. In this patient, the right temporal cranium was missing and the patient had 4×3 cm and 6×5 cm scalp defects. W...

Profunda Artery Perforator (PAP) Flap for Breast Reconstruction

Perforator flap breast reconstruction has strong appeal for many women undergoing mastec‐ tomies, but the procedure requires adequate donor site tissue. The most common donor site for perforator flap breast reconstruction is the abdomen, such as the deep inferior epigastric perforator (DIEP) or superficial inferior epigastric artery (SIEA) flap, as most women have adequate donor site tissue, th...

Contribution to the Anatomical Study of the Corticoperiosteal Flap of the Medial Femoral Condyle

OBJECTIVE to perform the anatomical study, in cadavers, of the corticoperiosteal flap of the medial femoral condyle, based on the medial genicular artery, evaluating challenges in dissection and the topographic patterns. MATERIALS AND METHODS fifteen limbs from eight cadavers were studied, ages ranging from 19 to 74 years old. They were placed at supine position, and a longitudinal incision o...
